The Young Parent and Youth Wellbeing program provides medium to high levels of support through a case management framework for young people and young families aged 8 - 21 years old, incorporating a planned approach to identification and assessment of their individual and family needs to increase their self-reliance, independence, health and wellbeing.

This program aims to assist in improving protective factors in place for young people. As well as addressing some of the risk factors and behaviors and reducing these risks through a variety of interventions, often requiring multi-disciplinary team/program and interagency approaches.  Support plans are tailor made specific to the client's needs.
