The Young Parent and Youth Wellbeing program provides medium to high levels of support through a case management framework for young people and young families 8 - 21 years old, incorporating a planned approach to identification and assessment of their individual and family needs to increase their self-reliance, independence, health and wellbeing.

Young Parent and Youth Wellbeing Program

The Young Parent & Youth wellbeing Program works with parents and young people aged 8 to 21 years, providing case managed supports, information, education & referral.

This Program aims to assist in improving protective factors in place for young people, whilst at the same time addressing some of the risk factors and behaviors and reducing these risks through a variety of interventions, often requiring multi-disciplinary team/program and interagency approaches.  Support plans are tailor made specific to the client's needs.
